Interchange Australia is a provider of several Hospital to Home options. Hospital to home packages are designed to facilitate a safe discharge home for people leaving a public hospital and prevent readmission to hospital by providing assistance under key support areas such as:
• Personal care
• Domestic Assistance
• Social/Shopping support
• Meal preparation/referral to meals on wheels
• In-home respite
Interchange Australia is a provider of Hospital and Home programmes including ComPacks and Healthy at Home. The ComPacks programme provides up to six weeks support to assist people discharged from a public hospital to recuperate at home and prevent readmission. The Healthy at Home programme also provides up to six weeks support to eligible people to prevent a hospital admission.
Interchange is also an approved provider of support services for people accessing a Transitional Aged Care Package (TACP) but only in certain areas. This enables people, following a hospital stay, to receive a combination of support services and nursing and allied health services for up to 12 weeks to facilitate physical and psychological rehabilitation goals.
Interchange Australia can also offer additional service options for for any of the above service types. If you are ineligible for either of the above packages, Interchange Australia can provide assistance to help you transition home safely.
