Interchange Australia provides a wide range of care and support services for people wanting to live independently in their own home. There are many benefits associated with in-home care services, both for the client and for their carers.
In-Home Support Benefits for Clients
The major benefit of in-home support is purely that it is in-home. Clients can remain in their own home and receive services in a private and comfortable setting. Numerous studies have demonstrated that individuals who receive services in their own home, recuperate faster than those receiving services in a health care facility. They are also less likely to be readmitted to hospital.
In-Home Support Benefits for Carers
Family members and friends are often the primary carer for family members who are infirm or aged. In-Home support services provide these carers with respite. While professional care providers deliver some of the services carers would normally perform, the carer is then able to focus on their daily lives and spend more quality time with their loved one.
Our In-Home Support Services include:
• Commonwealth Home Support Programme (CHSP)
Interchange Australia is an approved provider of CHSP services in the Wingecarribee Shire. CHSP services are available for people over 65 years of age and their carers who are mostly, but not completely, able to live and cope on their own. Interchange Australia is funded to provide personal care, domestic assistance, individual and group social support and respite.
• Home Care Packages Programme (HCP)
Interchange Australia is an approved provider of Home Care Packages. These packages provide older people, wanting to remain at home, with access to a combination of supports to assist with day-to-day activities. Support services include personal care, domestic assistance, gardening, home maintenance and modifications, social support and transport. The packages also include access to nursing, allied health and other clinical services, care coordination and case management.
• Hospital and Home
Interchange Australia is an approved provider of Hospital and Home programmes including ComPacks and Healthy at Home. The ComPacks programme provides up to six weeks support to assist people discharged from a public hospital to recuperate at home and prevent readmission. The Healthy at Home Programme also provides up to six weeks support to eligible people to prevent a hospital admission.
• Private Care
We provide a range of private services including assistance with showering and grooming, medication monitoring, cleaning, shopping, transport to appointments, accompanying people to social activities, respite, meal preparation, overnight support and hospital to home support.
